Job Duties

Monitor and report changes in service user circumstances to relevant professionals promptly.
Encourage participation in social, educational, and recreational activities to improve wellbeing.
Work collaboratively with families, carers, and external agencies to ensure coordinated support delivery.
Maintain accurate records while complying with organisational safeguarding and confidentiality policies.

Job Requirements

Minimum NVQ Level 2 or Level 3 in Health and Social Care or related qualification required.
At least one year of experience in care, residential support, or community-based services required.
Experience supporting individuals with mental health needs, learning disabilities, or challenging behaviour preferred.
Strong communication skills for engaging effectively with young people, families, and professionals.
Ability to work under pressure while managing complex and emotionally demanding situations effectively.
Strong empathy and relationship-building skills to support vulnerable individuals appropriately.
Ability to work independently, including visiting service users in community or home settings.
Good organisational and problem-solving skills to manage workloads and competing priorities efficiently.
Flexibility to work evenings, weekends, and respond to urgent situations when required.
