  • Ensure that patients’ case notes and referral information are recorded by the trust policy.
  • The person in this capacity must have a professional and private manner at all times.
  • The job will need face-to-face or phone interactions with patients, family, colleagues, nursing, and AHP professionals in health and social care.
  • Participate in the coordination of delayed discharge information from reports, overviews, and ward visits, as well as communication with ward coordinators to trigger warnings.
  • This enables qualified members of the Integrated Discharge Service Team to address discharge protocol delays.
  • Provide advice to ward personnel at all levels as discharge-related questions occur, within the scope of their expertise and responsibilities.
  • Refer lingering issues, disagreements, and care process delays to the senior complicated case manager to expedite their resolution.
  • Daily, visit wards to prioritize and manage one’s caseload, working independently to meet deadlines and deliver high-quality service.
  • Encourage discharge practices that align with the Hospital and Community Support Policy for 2021.

  • Respond to bleeps and calls professionally and promptly.
  • Referrals and trusted assessment documents should be entered and screened.

Person Specifications:

Essential

  • A minimum of GCSE level 4-9 in English and Mathematics, as well as NVQ or equivalent experience or training in an acute hospital setting.
  • Experience in discharge planning based on ward location.

  • Capability to handle a diverse caseload with competing priorities while completing projects within tight deadlines.
