Wiltshire Council is seeking compassionate and dedicated Support Workers to join its Reablement team in Salisbury, England. This is a full-time, skilled worker role offering a competitive hourly rate of £13.47 – £13.90 per hour.

About Role
As a Reablement Support Worker, you will play a crucial role in helping individuals regain independence in their daily lives following hospitalisation. Working closely with occupational therapists, you will assist in creating and implementing personalised reablement plans tailored to each individual’s needs.
This role requires flexibility to work on a rota basis between 7:00 AM and 10:00 PM, 365 days a year, demonstrating adaptability, teamwork, and a genuine passion for making a positive difference in people’s lives.

Responsibilities
- Support individuals to achieve their independence goals through personalised reablement plans
- Assist with daily living activities, such as personal care, mobility support, and household tasks
- Work closely with occupational therapists to monitor and adjust care plans as needed
- Communicate effectively with clients, families, and healthcare professionals
- Maintain accurate records and report on progress, incidents, and observations
- Uphold high standards of dignity, respect, and safeguarding in all interactions
- Contribute positively to a team environment while working independently when required
- Participate in training and development opportunities to enhance professional skills
Requirements
- Genuine passion for helping others and providing person-centred care
