Responsibilities

Undertake professional social work assessments and interventions for adults with care and support needs
Develop and implement person-centred care and support plans that promote independence and wellbeing
Manage a caseload in line with statutory duties, professional standards, and council policies
Work collaboratively with health professionals, families, and partner agencies to deliver coordinated support
Safeguard vulnerable adults in accordance with legal and ethical frameworks
Maintain accurate, timely, and compliant case records and reports
Participate in regular supervision, training, and continuous professional development activities

Requirements

Recognised Social Work qualification and current registration with Social Work England (SWE)
At least one year of experience working in Adult Social Care (for Skilled Worker sponsorship consideration)
Strong knowledge of UK social care legislation, safeguarding, and best practice frameworks
Full UK driving licence and willingness to drive (subject to Equality Act exceptions)
Confident communication skills and the ability to work in multidisciplinary environments
