We are currently looking for an experienced Health Care Assistant to join our Community Services team, providing nighttime support through Hospice at Home.

Job Overview

As a Night Health Care Assistant, you will enhance the quality of life for patients and their families. You will deliver personalized palliative care in their homes, performing clinical interventions under the guidance of registered nurses while upholding high standards of patient care.

Responsibilities

Care Provision:

  • Follow care plans and conduct risk assessments while providing individualized care.
  • After training, perform various clinical care interventions, including stoma care.
  • Make independent decisions to improve patient care quality.
  • Evaluate clinical health and safety risks.
  • Use sound judgment in emergencies.
  • Report changes in patients’ conditions to senior nurses.
  • Provide emotional support and coordinate referrals to hospice services.
  • Maintain patients’ privacy and dignity.
  • Execute last rites for deceased patients and confirm anticipated demise.
  • Prevent complications from immobility, such as pressure ulcers and falls.
  • Offer level-one medication management.
    Onboard new staff and share expertise.

Communication:

  • Build and maintain a positive reputation for the organization.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Recognize limitations in providing emotional support.
  • Uphold patient confidentiality.
  • Collaborate as a team member while following policies.
  • Maintain accurate documentation of care provided.

Health and Safety:

  • Prioritize patient safety and welfare consistently.
  • Identify patients at risk, including those prone to falls.
  • Contribute to health and safety risk assessments.
  • Follow incident reporting procedures.

Education:

  • Attend required training and service development opportunities.
  • Enhance knowledge and skills in palliative care.
  • Conduct annual self-evaluations and personal development assessments.
  • Serve as a role model for new staff.

Requirements

  • Previous experience as a Health Care Assistant or in a similar role.
  • Relevant qualifications in health or social care are preferred.
  • Strong communication skills, both verbal and written.
  • Ability to provide emotional support to patients and their families.
  • Commitment to maintaining patient confidentiality.
  • Flexibility to work night shifts and travel within the county.
  • Understanding of palliative care principles is advantageous.
  • Willingness to participate in ongoing training and development.
